Diabetes can damage the small blood vessels of the retina. This is diabetic retinopathy. In the early stages there is no pain and vision often stays clear, so many people find out only at an eye check.

How common is it?

According to India’s National Diabetes and Diabetic Retinopathy Survey 2015–19, 16.9% of people with diabetes aged 50 and above had diabetic retinopathy, and 3.6% had sight-threatening retinopathy. About one in three people with diabetes found in the survey did not know they had it.

Who should get checked

  • Everyone with diabetes, at least once a year, even with good sugar control and clear vision

  • Sooner if you have had diabetes for over 10 years, use insulin, have high blood pressure or kidney disease, or are pregnant with diabetes

  • Anyone with diabetes who notices blurring, floaters or patchy vision

Stages

  1. Non-proliferative (early) retinopathy: small bleeds and leaks. Needs monitoring and good sugar, BP and cholesterol control.

  2. Diabetic macular edema: fluid at the centre of the retina, which blurs reading vision. Treated with anti-VEGF injections.

  3. Proliferative retinopathy: fragile new vessels grow. Treated with in-house retina laser treatment where advised and/or injections.

Tests at your visit

Dilated retina examination, OCT scan (Huvitz OCT, in-house) to check the macula for swelling, and retinal photographs where useful. Bring your HbA1c, blood pressure readings and medicine list. Dilating drops blur vision for 4–6 hours, so please do not drive yourself home.

Treatment

  • Anti-VEGF injections for macular edema. Anti-VEGF medicines used at New Vision Eye Centre include Razumab, Eylea, Pagenax and Vabysmo. The choice depends on your condition, your scans and your cover, and your doctor will explain the options. Same-day injection after consultation is possible where suitable.

  • In-house retina laser treatment where advised, for proliferative retinopathy.

  • Teamwork with your physician on sugar, blood pressure and cholesterol, which protects the eyes too.

Can the damage be reversed?

Treatment aims to protect vision and slow progression. In the early stages, damage can be reversed. In advanced cases, damage is only partly reversible. This is why yearly checks matter.

Cashless options

Injections and retina treatment are cashless under CGHS, ECHS, DGEHS and health insurance, subject to approval.

Frequently asked questions

My vision is fine. Do I still need a check?

Yes. Diabetic retinopathy usually causes no symptoms until it is advanced.

Will cataract surgery affect my diabetic retina?

If you have diabetes, your retina is checked before cataract surgery and followed up afterwards, so any swelling can be treated early.
