Oculoplasty and eye aesthetics in Paschim Vihar: eyelid, tear duct and facial aesthetic surgery

Oculoplasty is surgery of the eyelids, the tear drainage system, the eye socket and the face around the eyes. It treats problems that affect comfort, vision and appearance, and includes aesthetic procedures such as blepharoplasty, Botox and fillers. At New Vision Eye Centre, oculoplasty and aesthetic procedures are performed by Dr Shilpa Taneja Mittal, DNB (Ophthalmology), MNAMS, who completed an 18-month fellowship in Orbit, Oculoplasty and Ocular Oncology at Aravind Eye Hospital, Madurai, and a 12-month fellowship in Facial Plastic and Aesthetic Surgery at The Esthetic Clinics, Mumbai.

Eyelid and tear duct surgery

Ptosis (droopy eyelid)
What it is: The upper eyelid sags and can cover part of the pupil. In adults it usually comes with age or long-term contact lens use; some children are born with it.
Treatment: Ptosis correction surgery, which tightens or shortens the muscle that lifts the eyelid.

Entropion
What it is: The eyelid turns inward, so the lashes rub against the eye, causing redness, watering and scratching of the cornea.
Treatment: Surgery to turn the eyelid back to its normal position.

Ectropion
What it is: The lower eyelid sags and turns outward, causing watering, dryness and red, exposed lining.
Treatment: Surgery to tighten and reposition the eyelid.

Blocked tear duct (watering eye)
What it is: Tears cannot drain into the nose, so the eye waters constantly and may have sticky discharge or a swelling near the nose.
Treatment: DCR (dacryocystorhinostomy), which creates a new tear drainage passage into the nose.

Eyelid lumps
What it is: Chalazion (stye-like lump), cysts and other growths on the eyelid.
Treatment: Excision, with the tissue sent for laboratory testing when needed.

Artificial eye
What it is: For an eye that has been lost, or is blind and shrunken, after injury or disease.
Treatment: Fitting of an artificial eye (ocular prosthesis) matched to the other eye, with advice on care.

Eyelid injuries
What it is: Cuts and tears of the eyelid after an accident.
Treatment: Repair to restore eyelid shape and function.

Eye and facial aesthetics

Blepharoplasty (eyelid surgery)
What it is for: Excess, hooded skin on the upper eyelids, or puffiness and bags under the eyes. When drooping skin blocks the upper field of vision, it is also a functional problem.
How it is done: Removal or repositioning of excess skin and fat through fine incisions hidden in the eyelid crease or below the lashes. Day-care surgery under local anaesthesia.

Botox (botulinum toxin)
What it is for: Crow's feet, frown lines and forehead lines. Also used medically for eyelid twitching and spasm (blepharospasm) and facial spasm.
How it is done: Small injections into specific muscles. Takes about 15 minutes; results appear over a few days and usually last 3–4 months.

Dermal fillers
What it is for: Hollows under the eyes, deep folds, and lost volume in the face.
How it is done: Injections of a gel (usually hyaluronic acid) to restore volume. Results are seen immediately and last several months, depending on the area and the filler used.

Other facial aesthetic procedures
What it is for: Discussed individually at consultation.
How it is done: Planned after examination, with realistic expectations explained.

All aesthetic treatments begin with a consultation to understand your concerns, examine the area and explain what can and cannot be achieved.

Ptosis in children

A droopy eyelid in a child is more than a cosmetic concern. If the eyelid covers the pupil, the eye may not develop normal vision (lazy eye). Children with ptosis need an eye examination early, and surgery may be advised sooner if vision is at risk.

Watering eyes in babies

Many babies are born with a tear duct that has not fully opened. It often opens by itself in the first year, helped by gentle massage shown by the doctor. If it does not, a simple probing procedure usually clears it. DCR is mainly needed in adults.

What to expect

Most oculoplasty and blepharoplasty procedures are day-care surgery under local anaesthesia; you go home the same day. Healing usually takes one to two weeks, and stitches, if any, are removed at a follow-up visit. Botox and fillers need no recovery time, and most people return to their routine the same day. Your treatment plan is discussed in detail before you decide.

Frequently asked questions

Is ptosis surgery cosmetic?

When a droopy eyelid blocks vision, surgery is a medical treatment and is often covered by insurance. Purely cosmetic eyelid surgery is usually not covered.

Will DCR stop my eye from watering?

DCR usually relieves watering caused by a blocked tear duct. The cause of watering is checked first, because watering can also come from dry eye or eyelid problems.

Is blepharoplasty covered by insurance?

Only when excess eyelid skin is shown to block vision. Blepharoplasty for appearance, Botox and fillers for cosmetic reasons are not covered by insurance or CGHS.

Are Botox and fillers safe?

In trained hands, with genuine products and correct doses, they are well-established treatments. Your consultation covers what to expect before any treatment.

How long does an artificial eye last?

With good care it lasts several years. It is checked and polished periodically and replaced when it no longer fits well.

Is oculoplasty covered under CGHS or insurance?

Many procedures, including DCR, entropion, ectropion and ptosis surgery that affects vision, are covered under CGHS, ECHS and health insurance, subject to approval. Call the insurance team on 96251 54575.
